Benefit Descriptions ¡G
I) Accidental Death Benefits ¡V pays the insurance amount for loss of life due to accident.

If the accidental death is incurred while the insured member is riding as a fare-paying passenger in any public common carrier or is a pedestrian injured in a traffic accident and / or struck by any motor driven/powered vehicle; or the death is resulted from flooding and/or landslide in Hong Kong SAR caused by natural disasters, indemnity of double the insurance amount for Accident Death Benefit (inclusive of Accidental Death Benefit) will be payable.  
  (a) Accidental death benefit HK$ 100,000

 

(b) Accident death benefit (Double Indemnity) HK$200,000
  *Maximum coverage for (a) and (b) is HK$200,000
   
II) Dental Benefits ¡V provides the dental services through appointed dental clinics.
  Treatment to be covered:
  Dental examination (unlimited)  
  Polishing & scaling (once a year)*  

 

 Simple composite (white) fillings for anterior teeth and
malgam (silver) filling for posterior teeth (carious)** (unlimited)		  
  Simple extractions (unlimited)***  
  Preventive dental fluoride treatment (once a year)  
  X-Rays (small intra-oral)****

Pain Treatment:  
  Drainage of abscesses (without surgery)

 (a) Re-condition of old fillings, evacuation of degenerated dentine and pre-treatment sterilization procedure are not included.
(b) Coverage does not included treatment on any single tooth with decay more than one surface of a tooth.

 Simple extraction means extraction of tooth, which is clearly mobile due to Periodontal Disease.
Excluded:
(i) Wisdom teeth
(ii) Extractions due to decay
(iii) Extractions due to fracture.
(iv) Serial extractions or extractions related to orthodontic treatments.

 Duplicate of X-ray is not included. If required, there will be a charge of $100 each.
(2) Treatments or operations to be performed in hospitals will not be covered by this dental plan.

Procedure for enjoying our check-up services :
1. Applicant should complete the application form and fax it to Canadian, or using our online application service for apply.
2. Applicant should receive confirmation of acceptance from us through telephone or e-mail.
3. Applicant should receive the membership card by mailing and the normal commencement date of the membership card should be on the first and sixteen of each month.
 
Payment methods :

2. By deposit : Deposit fee to our Hang Seng Bank Account 275-083889-668 and fax the pay-in-slip to 81671003.
